Job Summary
The objective of this position is to provide Housing Focused and client driven case management services to formerly homeless individuals. This will be a short-term rental assistance program, with case management designed to help clients become self-sufficient and able to pay rent on their own at the end of the program. Above all else Case Managers seek to assist clients in achieving stability in housing by connecting them with the resources they need to achieve their personal goals.

JOB DUTIES
Work with a Case Load
You Will
- Assisting our diverse population of guests on your set caseload to establish goals.
- Assist tenant in obtaining employment and or working on applying for Social Security benefits.
- Focus on housing and strengths-based practices to meet the unique needs of each tenant.
- Assess children’s needs to make appropriate community referrals and liaise with schools as needed for supportive services
- Daily collaboration with all departments to promote a healthy team environment.
- Seeking out, engaging, and networking with community partners to find housing options and additional resources.
- Attended weekly team meetings and contribute to case conferencing conversations.
- Keep accurate records, case management notes, and statistical data as required and participate in special projects as needed.
Resource Collaboration
You Will
- Provide direct, trauma-informed case management services to guests, including outreach, ongoing assessments, goal setting, and creative problem solving, using a network of community resources.
- Collaborate with the crisis clinician and others to find ways to support individuals with mental and behavioral health barriers.
- Ability to accept direction and constructive feedback from Supervisor.

Qualifications:
- Fluent in English and Spanish
- Ability to communicate effectively with diverse audiences
Education and Experience
- Experience working or volunteering with diverse and vulnerable populations.
- Lived, work or volunteer experience preferred, or a degree in a related field.
Physical and Equipment Requirements
- Ability to lift 25 pounds.
- Ability to stand and walk for at least an hour at a time.
- Ability to drive a vehicle.
- Ability to pass a pre-employment background check and drug screen.
